The use of Maternal and Child Health (MCH) books as extension materials for pregnant women is a crucial aspect of public health services. These books serve as a health record for mothers and children, containing vital information and counseling materials on nutrition, pregnant women cards, and health services. However, despite being distributed to all Puskesmas in Central Aceh Regency, the use of MCH books in extension activities has been suboptimal. This study aims to analyze the influence of midwives' characteristics and Puskesmas organizational factors on the use of MCH books.

The MCH book is an essential tool in public health, providing a comprehensive record of a mother's and child's health. The book contains information on nutrition, pregnant women cards, and health services, making it an invaluable resource for midwives and pregnant women. However, the initial survey in Central Aceh Regency revealed that many materials in the book are not used as a reference in counseling to pregnant women. This study seeks to understand the factors that contribute to the suboptimal use of MCH books.

This study employed a cross-sectional design, involving 208 midwives selected randomly. The data obtained were analyzed using multiple logistics regression. The study aimed to examine the influence of midwives' characteristics, including knowledge, motivation, workload, working period, and training in the use of MCH books, as well as Puskesmas organizational factors, such as work climate, supervision, and leadership, on the use of MCH books.

The results of the analysis showed that there was a significant effect of the knowledge variable (p = 0.018), motivation (p = 0.036), training in the use of MCH books (p = 0,000), supervision (p = 0.037), and leadership (p = 0,000) on the use of MCH books. However, workloads and working periods did not show a significant effect. The most influential factor was training in the use of MCH books, with a coefficient value of B = 3,106, indicating the importance of training in improving the ability of midwives.

The findings of this study provide important implications for the Central Aceh District Health Office. The results suggest that sustainable training, especially regarding the use of MCH books, is essential to increase midwife's knowledge and skills in providing counseling to pregnant women. Additionally, building a reward and punishment system can motivate midwives to be more active in utilizing MCH books. Routine supervision of the use of MCH books, including distribution to pregnant women, is also necessary to ensure that MCH books function optimally as a counseling tool.

  3. Routine Supervision: Routine supervision of the use of MCH books, including distribution to pregnant women, is necessary to ensure that MCH books function optimally as a counseling tool.

This study has several limitations. Firstly, the study was conducted in Central Aceh Regency, and the findings may not be generalizable to other regions. Secondly, the study only examined the influence of midwives' characteristics and Puskesmas organizational factors on the use of MCH books, and did not explore other factors that may contribute to the suboptimal use of MCH books.
