The Influence of Achievement Motivation and Therapeutic Communication Skills on Nurses' Performance in Nursing Care at Kabanjahe General Hospital in 2009
Introduction
The Kabanjahe General Hospital, located in Karo Regency, plays a vital role in providing health services to the community. Despite its importance, the hospital's performance reports over the past three years (2005-2007) showed a Bed Occupancy Rate (BOR) below the national target of 80%. The length of patient care (Length of Stay or LOS) was also recorded at 6-9 days, which is higher than the national average. This study aims to analyze the effect of achievement motivation and therapeutic communication skills on nurses' performance in nursing care at Kabanjahe General Hospital.
Methodology
This study employed a survey with an explanatory research type, involving 126 nurses as a population. Of these, 60 nurses were selected using purposive sampling to be sampled. Data analysis was carried out using multiple linear regression tests with a 95% confidence level. The data used in this study consisted of a score of achievement motivational variables (X1), therapeutic communication skills (X2), and nurse performance scores (Y). The instrument used to collect data was in the form of questionnaires that had been validated by experts and tested to find out their reliability and validity.
Results
The results showed that of the four variables tested, namely the need for achievement, the need for affiliation, the need for power, and therapeutic communication skills, only two variables had a significant influence on the performance of nurses. This variable is the need for power with a significance of 0.018 and knowledge of therapeutic communication capabilities with a significance of 0.012. Conversely, the variable need for achievement (significance 0.235) and the need for affiliation (significance 0.760) does not show a significant effect on the performance of nurses in the Kabanjahe General Hospital.
Discussion
Achievement motivation is an individual internal drive to achieve goals and do the best at work. In the context of nurses, this motivation plays an important role in improving the quality of health services. However, the results showed that the need for achievement and affiliation did not have a significant impact. This may be caused by work culture in hospitals that emphasize more on teamwork and individual autonomy in decision making.
Therapeutic communication capabilities, on the other hand, are proven to have a significant influence on the performance of nurses. This indicates that nurses who are able to communicate well, both with patients and other medical teams, can provide higher quality care. Effective communication helps in reducing errors in nursing care, increasing patient satisfaction, and strengthening the relationship between nurses and patients.
Conclusion
The results of this study suggest that knowledge of therapeutic communication skills is the dominant factor that affects the performance of nurses in the Kabanjahe General Hospital. Therefore, hospitals need to consider training and development of therapeutic communication skills for nurses. In addition, creating a work environment that supports achievement motivation through appreciation or recognition of good performance can help improve nurse performance as a whole.
Recommendations
Based on the findings of this study, the following recommendations are made:
- Hospitals should provide training and development programs for nurses to improve their therapeutic communication skills.
- Hospitals should create a work environment that supports achievement motivation through appreciation or recognition of good performance.
- Hospitals should design a more effective program to improve the quality of health services, which in turn has a positive impact on patients and the community.
Limitations
This study has several limitations. Firstly, the sample size was relatively small, which may not be representative of the entire population of nurses in the Kabanjahe General Hospital. Secondly, the study only focused on the effect of achievement motivation and therapeutic communication skills on nurses' performance, and did not explore other factors that may influence nurse performance.
Future Research Directions
Future research should aim to explore other factors that may influence nurse performance, such as work environment, leadership style, and organizational culture. Additionally, future research should aim to develop and evaluate interventions that can improve nurse performance and patient outcomes.
