Introduction

The efficiency assessment of health services in hospitals is a crucial aspect of determining how well the available resources, including beds, are utilized. In Doloksanggul Regional General Hospital (RSUD), Humbang Hasundutan Regency, the efficiency of inpatient services can be analyzed using Barber Johnson charts. This study aims to identify the efficiency of inpatient services in the hospital, which in recent years shows results below the standards set by the Ministry of Health.

Background

The efficiency of inpatient services in Doloksanggul Regional Hospital has been a concern in recent years. The data on the efficiency of inpatient services from the hospital shows unsatisfactory developments. In 2014, the Occupancy Rate (BOR) bed value was only 13.44%, with an average length of stay (LOS) 2.1 days, Time of Inpatient (TOI) 13.46 days, and Bed Turn Over (BTO) 23.46 times. In 2015, the drill increased to 16%, and LOS to 2.5 days, but still far from ideal. 2016 showed a slight improvement with a 26.6% drill and 2.78 days, but did not meet efficiency standards. This indicates the existence of challenges in providing optimal health services in the area.

Methodology

This research uses a qualitative approach to collect data from in-depth interviews with five informants, including the Head of Services, Doctors, Nurses, Medical Record Officers, and inpatients. Secondary data are also obtained from hospitals to support analysis. The results showed that the efficiency of inpatient services in Doloksanggul Regional Hospital was not optimal. The number of beds available is not in accordance with the number of patients treated, and the average day of nursing is also still low.

Factors Influencing Efficiency

Some factors that influence the efficiency of services in this hospital include limited health facilities, especially medical support facilities, as well as lack of health workers, especially specialist doctors and nurses. This limitation has a direct impact on the ability of the hospital in providing quality services to patients.

Results

The results of this study confirmed that Doloksanggul Regional Hospital in 2017 was not included in the efficient category according to the Barber Johnson chart. The efficiency of inpatient services in the hospital was not optimal, and several factors contributed to this inefficiency.

Discussion

The findings of this study highlight the importance of attention to the development of human resources and infrastructure in an effort to improve the quality of services in hospitals. The hospital's limited health facilities and lack of health workers, especially specialist doctors and nurses, have a direct impact on the ability of the hospital in providing quality services to patients.

Conclusion

In conclusion, this study confirms that Doloksanggul Regional Hospital in 2017 was not included in the efficient category according to the Barber Johnson chart. To improve service efficiency, several strategies that can be applied include increasing the quantity of health workers by increasing the number of specialist doctors and nurses, completing medical devices supporting inpatient services, adjusting the number of beds with patient needs, and increasing the average treatment day.

Recommendations

To improve the efficiency of inpatient services in Doloksanggul Regional Hospital, the following recommendations can be made:

  • Increase the quantity of health workers by increasing the number of specialist doctors and nurses.
  • Complete medical devices supporting inpatient services.
  • Adjust the number of beds with patient needs.
  • Increase the average treatment day.

Limitations

This study has several limitations, including the use of a qualitative approach and the limited number of informants. Future studies can use a quantitative approach and include a larger number of informants to provide more comprehensive results.

Future Research Directions

Future research can focus on the implementation of the strategies recommended in this study to improve the efficiency of inpatient services in Doloksanggul Regional Hospital. Additionally, future studies can explore the impact of these strategies on the quality of services provided to patients.
