Introduction

Despite having a good Safety and Health (SMK3) management system in place, the company still experiences work accidents. This study aims to provide an overview of the application of the Occupational Safety and Health Management System (OSHMS) at the Palm Oil Factory Kisaran PT. Bakrie Sumatera Plantations Tbk in 2016.
Background
The palm oil industry is a high-risk industry, with workers exposed to various hazards such as machinery, chemicals, and physical injuries. The Occupational Safety and Health Act (OSHA) requires employers to provide a safe and healthy work environment for their employees. The SMK3 management system is a set of guidelines and procedures that aim to prevent work accidents and ensure the health and safety of employees. This study uses a descriptive observation method with a population consisting of all employees, namely 85 people, taken through purposive sampling techniques. The results showed that the level of achieving the application of SMK3 was based on 166 SMK3 audit criteria at the Palm Oil Mill in 2016 recorded the following results:

- Principle 2: K3 Planning - 22 Criteria (13.2%)
- Principle 3: Implementation of K3 Plans - 66 Criteria (39.8%)
- Principle 4: Monitoring and Evaluation of K3 Performance - 24 Criteria (13.9%)
- Principle 5: Review and Improvement of K3 Performance - 23 Criteria (13.8%)
Results
The results showed that the highest achievement is in the principle of implementing K3 plans with 66 criteria that are exceeded, while the lowest achievement is in the principle of determining OSH policy. In the past three years, there were 14 work accidents in this factory. Although the level of achievement of the implementation of SMK3 has met 160 criteria (96.4%), the number of accidents that still occur indicate weaknesses in aspects of implementation and supervision in the field.

Achievement of the high application of SMK3 does not necessarily indicate that the risk of work accidents has been fully resolved. In the context of the palm oil mill PT. Bakrie Sumatera Plantations Tbk, several factors may be the cause of accidents, including:
