Improving Patient Safety: Analysis of the Implementation of Patient Safety Targets in the Inpatient Room of the University of North Sumatra Hospital in 2019

Patient safety is a critical concern in healthcare institutions worldwide. The University of North Sumatra Hospital (RSUSU) has implemented a patient safety service system, but unexpected events related to medical errors still occur. In 2018, there were 5 cases of unexpected incidents (KTD), 12 cases of potential injuries (KPC), 4 cases of almost injury (KNC), and 1 case of Sentinel. To better understand the implementation of patient safety goals (CSWs) in RSUSU, a qualitative study with thematic analysis was conducted in 2019. This study involved 4 heads of nurses and 4 implementing nurses in the inpatient room.

The implementation of the third target of CSWs, namely increasing the safety of drugs, is in accordance with the standards. 2. Patient Identification

There are weaknesses in the implementation of the accuracy of patient identification. Low awareness and compliance of nurses to carry out tasks according to the standard operating procedures (SOP) identification of patients is a major factor. Communication between doctors and nurses has not been fully in accordance with CSW standards. Some nurses forget to do "read back", so the dissemination of information is not optimal. Recommendations

Based on the results of the study, the RSUSU management needs to improve evaluation and monitoring of the implementation of CSWs in accordance with applicable SOPs. Some recommendations that can be applied:
