Background

The importance of accurate and complete documentation of nursing care cannot be overstated in the healthcare industry. Regulations on medical records and nursing practices require nurses to document every nursing care service provided to patients. However, the results of a preliminary study at Dr. Zainoel Abidin Province of Nanggroe Aceh Darussalam shows that the level of application of nursing care documentation only reaches 24.32%. This condition indicates that the documentation of nursing care by nurses in the inpatient installation of Dr. Zainoel Abidin has not met the standards set by the Ministry of Health of the Republic of Indonesia.

Objective

This study aims to examine the influence of individual characteristics, psychological factors, and organizational factors on documenting nursing care at Dr. Zainoel Abidin. The primary objective is to identify the factors that contribute to the low level of nursing care documentation and to provide recommendations for improvement.

Method

This study uses an explanatory survey approach conducted in July to October 2008. The research sample involved all nurses who served in the inpatient installation, totaling 289 people. Data were analyzed using multiple linear regression tests with a significance level of 5%. The study aimed to examine the relationship between individual characteristics, psychological factors, and organizational factors and the documentation of nursing care.

Results

The results showed that the variables of individual characteristics (working period), psychological variables (motivation and perception of work), and organizational variables (rewards, leadership, and job design) together had an influence on documentation of nursing care at 65.3%. Of the three groups of variables, motivation is the dominant factor that most influences the documentation of nursing care.

High motivation will trigger nurses to be responsible for their duties and carry out complete and accurate documentation. In addition, perception of work also has a significant effect. Nurses who have a positive perception of work tend to be more motivated to provide the best service, including in terms of documentation. A positive perception of work can lead to increased job satisfaction, which in turn can motivate nurses to document their care more accurately.

3. Effect of Organizational Factors

A fair and transparent reward system will motivate nurses to work better, including in documentation. Effective leadership will create a positive work culture and encourage nurses to follow the established documentation standards. Ergonomic and supportive job designs will facilitate nurses in documenting. A well-designed job can reduce the physical and mental demands of documentation, making it easier for nurses to complete their tasks accurately and efficiently.

Recommendation

Based on the results of the study, the following are recommendations that can be applied by the management of Dr. Zainoel Abidin:

  • Increased internal policy: Establish internal policies and rules regarding the implementation of operating procedures (SOP) for nurses in inpatient installations to support a better documentation system.
  • Adjustment of the remuneration system: Pay attention to aspects of reward, especially the remuneration system that should be determined based on workload.
  • Improving Leadership Management: Improving the Nursing Leadership Management System, especially the role of evaluation and monitoring periodically and continuously. This is important to minimize deficiencies in documentation and improve the quality of documentation of nursing care.

Conclusion

This study shows that individual characteristics, psychological factors, and organizational factors have a significant influence on documenting nursing care. Motivation is a dominant factor that encourages nurses to document well. Therefore, the management of RSUD Dr. Zainoel Abidin needs to take the right steps to improve the quality of nursing care documentation by taking into account the factors that have been identified in this study.

The study was conducted in a single hospital, which may limit the generalizability of the findings to other hospitals. Additionally, the study relied on self-reported data from nurses, which may be subject to biases and errors.
