Introduction
The quality of health services in hospitals is often measured through data contained in the patient's medical record. A good and complete medical record is an important indicator of quality health services. However, the initial survey conducted in three hospitals owned by PTPN IV showed that the completeness of the filling of the medical record was far from the standards set by the Ministry of Health. The survey results show that Pabatu Hospital only has 18% completeness, 26% Laras Hospital, and Balimbingan Hospital is 11%. This incompleteness generally lies in the note column that should be filled by doctors who take medical actions. This study aims to analyze how the characteristics of an individual doctor and extrinsic motivation affect the performance of a doctor in completing the filling of medical records for inpatients.
Background

Methodology
This study is an explanatory research, with a population consisting of 45 doctors with fixed power status, honorary, and consultants. Data was collected through interviews and direct observations in the hospital, and then analyzed using multiple linear regression tests. The results showed that there were two variables that had a significant effect on the performance of doctors in the completeness of the medical record filling, namely working conditions (p = 0.001) and supervision (p = 0.047), both had a P value smaller than 0.05.
Working Conditions and Doctor Performance

Comfortable workspace, adequate medical equipment, and a supportive environment, can increase the focus and efficiency of doctors in carrying out their duties. In addition, having a clear organizational chart in accordance with the Ministry of Health's standards and forming a medical committee in every hospital can help in overseeing and controlling better medical records.
