Introduction

The problem of health behavior that is often found in Indonesia is a habit of defecation carelessly (BABS). This behavior is not only a concern for individual health but also has a significant impact on the environment. According to data from the Mandailing Natal District Health Office in 2014, around 60.41% of families in the area were still applying BABS habits. This condition is very alarming and requires immediate attention from the community and health agencies.

The Importance of Counseling

Counseling is an essential tool in changing health behavior. It provides individuals with the knowledge and skills necessary to adopt healthy habits. In the context of BABS, counseling can help community leaders understand the importance of proper defecation habits and how to implement them in their daily lives. The purpose of this study was to examine the effect of counseling using lecture and discussion methods on the knowledge and attitudes of community leaders in Simpang Durian Village and Batu Gajah Village, Lingga Bayu District, Mandailing Natal Regency.

Methodology

This study applies the Quasi Experiment method with a pretest-posttest group design. The population involved 60 family heads in Simpang Durian Village and 54 family heads in Batu Gajah Village. The data obtained were analyzed using statistical tests such as Paired T-Test and Wilcoxon Independent T-Test.

The Effect of Counseling

The results of the analysis show that there is a significant influence of counseling with the lecture method on knowledge (p = 0.001) and attitudes (p = 0.001) community leaders. In addition, counseling using the discussion method also shows a significant effect on knowledge (p = 0.001) and attitudes (p = 0.001) of the community about BABS. Interestingly, the discussion method is proven to be more effective than the lecture method in increasing knowledge (p = 0.030) and attitude (p = 0.029) community.

Discussion

Further analysis shows that the discussion method creates a more active interaction between extension workers and participants, thus facilitating a deeper understanding of the problems encountered. Discussion allows the community to share experiences, create awareness, and provide mutual support in overcoming babs habits. In this case, community involvement in the counseling process is very important, because they can become effective agents of change in their environment.

The Role of Community Leaders

Community leaders play a crucial role in promoting healthy behavior among their members. They can work together with the Puskesmas in motivating the community to adopt proper defecation habits. This collaboration can be done by designing programs that support behavior changes, such as routine counseling, environmental cleanliness training, and the provision of adequate sanitation facilities.

In conclusion, counseling with the lecture and discussion method is proven to have a positive influence on the knowledge and attitudes of the people regarding defecating carelessly. The use of discussion methods has a more significant impact, so it needs to be the first choice in the implementation of extension programs in the community. Joint efforts between community leaders and health agencies will be very helpful in overcoming this health problem.

Recommendations

Based on the findings of this study, the following recommendations are made:
