Introduction
Gout is a common health problem in Indonesia, with a prevalence of 24.7%. The increase in uric acid levels over two years not only causes gout but also has the potential to cause other diseases, including kidney disorders that occur in 60% of sufferers. To overcome this problem, traditional medicine has been used for centuries, and one of the plants that has been proven to have antihyperurisemia properties is the Jamblang leaf (Syzygium cumini L. (Skeels)). This study aims to evaluate the effect of giving ethanol extract from Jamblang leaves in reducing uric acid levels and providing protection to the kidneys to rats induced by oxonate potassium.
Background of the Study
Gout is a metabolic disorder characterized by the accumulation of uric acid in the body, which can cause kidney damage and other health problems. The increase in uric acid levels is caused by the imbalance between the production and excretion of uric acid. In Indonesia, gout has a high prevalence, and the increase in uric acid levels can cause kidney disorders in 60% of sufferers. To overcome this problem, traditional medicine has been used for centuries, and one of the plants that has been proven to have antihyperurisemia properties is the Jamblang leaf (Syzygium cumini L. (Skeels)).
Materials and Methods
In this study, 30 rats were used which were divided into six groups. One group is a normal control group (without treatment), while the other five groups are induced by oxonate potassium dose of 250 mg/kg body weight intraperitoneal. The induced group was then further divided into a group that was given a carboxy methyl cellulose (CMC) 0.5%, and ethanol extract of Jamblang leaves with a dose of 100 mg/kg BW, 200 mg/kg BW, and 400 mg/kg BW. As a comparison, allopurinol is given at a dose of 10 mg/kg BW. Each group consists of five male mice given orally treatment for seven consecutive days. On the eighth day, these mice are sacrificed through neck dislocation to be taken blood and kidney, which is then macroscopic and microscopic examined.
Results
The results of the analysis using the anova method and the HSD tukey test showed that the administration of ethanol extract of jamblang leaves at a dose of 100, 200, and 400 mg/kg BW had a significant effect in reducing uric acid levels, and improving kidney health based on the value of ureum, creatinine, and relative weight Kidney, when compared to the negative control group (p <0.05). The best doses obtained from this study were ethanol extract of Jamblang leaf 400 mg/kg BW, which succeeded in reducing uric acid levels by 55.8%, ureum levels 46.29%, creatinine levels 52.58%, and relative kidney weight 25, 53%. In addition, the histopathology of the kid's kidneys given by ethanol extract showed a significant improvement compared to the negative control group.
Discussion
The results of this study showed that the ethanol extract of Jamblang leaves at doses of 100, 200, and 400 mg/kg BW is effective as antihydeurisemia and has a nephroprotective effect, making it an alternative therapy that promises in the management of high uric acid levels. The active compounds in Jamblang leaves, such as MyRcetin, Kaempferol, and Quercetin, are believed to be responsible for the antihyperurisemia and nephroprotective effects. Further research is very necessary to explore the mechanism of action of active compounds in jamblang leaves and the potential application in clinical treatment.

The use of Jamblang leaves in traditional medicine can be considered as a safe and effective solution to overcome high uric acid problems and maintain kidney health. The ethanol extract of Jamblang leaves at doses of 100, 200, and 400 mg/kg BW is effective as antihydeurisemia and has a nephroprotective effect, making it an alternative therapy that promises in the management of high uric acid levels.
