Introduction

In the National Health Insurance (JKN) era carried out by the Social Security Organizing Agency (BPJS) since 2014, promotive and preventive health efforts are very important. This is because the funds allocated for health services can be in vain if they are not accompanied by efforts to prevent and improve public health. The implementation of promotive and preventive services is crucial in achieving optimal health outcomes and reducing the burden on the healthcare system.

Background

The Helvetia Puskesmas Medan City is one of the health centers that have been implementing promotive and preventive services in the JKN era. This research aims to analyze the implementation of promotive and preventive services at the Helvetia Puskesmas Medan City in 2016. Through a qualitative approach and interviews with six informants, this study aims to identify the challenges and suggestions to improve the quality of promotive and preventive services.

Methodology

This study used a qualitative approach and conducted interviews with six informants, including the Health Promotion Staff of the Medan City Health Office, the Head of the Helvetia Health Center, the Medan City BPJS staff, the Head of the Helvetia Health Center Health Promotion, and two JKN participating patients. The data collected from the interviews were analyzed using thematic analysis.

Results

The results showed that the Helvetia Puskesmas had run promotive and preventive programs both before and after the application of JKN. Public health efforts carried out include dental and oral health counseling, education to pregnant women, counseling about balanced nutrition, mosquito control causes of dengue fever (DHF), as well as counseling to the elderly after monthly exercise. On the other hand, individual health efforts are also carried out through the home visit of TB patients, home visits for handling malnutrition, and counseling about non-communicable diseases.

Challenges and Suggestions to Improve the Quality of Promotive and Preventive Services

Although the promotive and preventive programs at the Helvetia Health Center have been running, this research also identifies several challenges that need to be overcome. The quality of health workers handling promotive and preventive services needs to be improved through more intensive training and education. This is important to increase the effectiveness of programs and achieve more optimal public health goals. In addition, it is necessary to increase the motivation and alertness of the Helvetia Health Center Health Center in providing promotive and preventive services. This can be done by giving appreciation of good performance and creating a conducive work environment to run the program with full dedication.

Conclusion

This study shows that the Helvetia Medan Health Center has run promotive and preventive programs in the JKN era. However, to improve the quality of service and achieve a higher health degree of public health, efforts need to be made to improve the quality of health workers and motivate them to be more active in running promotive and preventive programs. It is hoped that this research can be a reference for the Medan City Health Office and the Head of the Medan Helvetia Puskesmas to improve the quality of promotive and preventive services at the Helvetia Puskesmas.

Based on the findings of this study, the following recommendations are made:
