Analysis of Patient Safety Implementation at Deli Medan General Hospital: A Critical Examination

Introduction

Patient safety is a critical aspect of healthcare that has gained significant attention in recent years. The implementation of patient safety programs is essential to reduce unwanted events that should be prevented. In Indonesia, the Ministry of Health has issued regulations to ensure that every hospital and health worker in charge carries out patient safety programs. Deli Medan General Hospital, like many other hospitals in Indonesia, has adopted patient safety as a top priority since 2012. However, despite the efforts, there are still many obstacles that arise in its implementation. This study aims to describe the implementation, monitoring, and factors that influence the implementation of patient safety programs at Deli Medan General Hospital.

Background

Patient safety is a critical aspect of healthcare that has gained significant attention in recent years. The World Health Organization (WHO) defines patient safety as "the reduction of harm to patients and minimization of risk of unnecessary harm associated with healthcare." The implementation of patient safety programs is essential to reduce unwanted events that should be prevented. In Indonesia, the Ministry of Health has issued regulations to ensure that every hospital and health worker in charge carries out patient safety programs. The policy issued by the Minister of Health of the Republic of Indonesia through Minister of Health Regulation No. 1691/MENKES/PER/VIII/2011 asserted that every hospital and health workers in charge must carry out patient safety programs.

Methodology

This study uses a qualitative approach with the purposive sampling method to choose eleven relevant informants. Data is collected through observation and in-depth interviews, then analyzed qualitatively and narratively. The main focus of this research is the fulfillment of documents related to the patient safety program, bearing in mind that the policy is still applied in this hospital.

Results

The results showed that the fulfillment of documents related to patient safety is still less than 80%. In addition, the socialization of guidelines, guidelines, and implementation instructions relating to patient safety has not been carried out thoroughly in all units. Provision of facilities, facilities, and infrastructure is also far from adequate. Although this program has received support from the Hospital Director, the realization of the support is still considered less optimal. One key factor that inhibits the implementation of patient safety is the less than the maximum role of the Hospital Patient Safety Team (TKPRS).

Discussion

The results of this study highlight the challenges faced by Deli Medan General Hospital in implementing patient safety programs. The less than 80% fulfillment of documents related to patient safety is a concern, as it indicates that the hospital is not fully compliant with the regulations. The socialization of guidelines and implementation instructions is also a critical aspect of patient safety, and the fact that it has not been carried out thoroughly in all units is a concern. The provision of facilities, facilities, and infrastructure is also far from adequate, which can hinder the implementation of patient safety programs.

The role of the Hospital Patient Safety Team (TKPRS) is also a critical aspect of patient safety. The team is responsible for socializing, coordinating, monitoring, and evaluating the implementation of patient safety in each unit. However, the results of this study show that the team is not playing a maximum role in the implementation of patient safety. This is a concern, as the team is essential in ensuring that patient safety is implemented effectively.

Conclusion

Analysis of the implementation of patient safety programs at Deli Medan General Hospital shows that although there is support from management, there is still much that needs to be improved in terms of fulfilling documents, socialization, and providing facilities. Increasing the role of TKPRS and collaboration between units is needed to create a safer environment for patients. An effective patient safety program not only has an impact on the quality of health services, but also increases public confidence in health facilities in Indonesia.

Recommendations

Based on the results of this study, the following recommendations are made:

  1. The Hospital Director should be more active in determining policies that support the patient safety program.
  2. The provision of facilities, facilities, and infrastructure should be improved to support the implementation of patient safety programs.
  3. The Hospital Patient Safety Team (TKPRS) should play a more active role in socializing, coordinating, monitoring, and evaluating the implementation of patient safety in each unit.
  4. All units involved should coordinate with each other and carry out operational procedures (SOP) standards related to patient safety.

Limitations

This study has several limitations. The sample size is small, and the study was conducted in only one hospital. The study also relied on qualitative data, which may not be generalizable to other hospitals. However, the study provides valuable insights into the challenges faced by Deli Medan General Hospital in implementing patient safety programs.

Future Research

Future research should focus on the implementation of patient safety programs in other hospitals in Indonesia. The study should also explore the role of the Hospital Patient Safety Team (TKPRS) in implementing patient safety programs. Additionally, the study should examine the impact of patient safety programs on the quality of health services and public confidence in health facilities in Indonesia.
